Health transitions for young people with life-limiting conditions

A research team based at the University of York is conducting a study of good practice in health services for young people and young adults with life-limiting conditions including :

  • supporting the transfer from paediatric to adult services.
  • condition management
  • acute admissions
  • symptom management
  • psychological, social, emotional, spiritual or family support
  • end of life care
